Ovulation management (OM) involves the use of fertility drugs to stimulate the ovaries to produce multiple eggs. There are two levels of ovulation medication. Clomiphene has been used since the early 1960s to stimulate ovulation. It is a pill that works with the brain to send a signal to the ovary to induce ovulation. The side effects are mild and the risk of twins is increased from 3 to 10 percent. The risk of triplets or higher is no greater than normal.
The second level of ovulation medication is gonadotropins. These medications are only available in injectable form and need to be taken on a daily basis. Monitoring with sonograms and hormone levels is necessary to keep careful track of the maturing follicles. Such monitoring is usually performed in the morning so that blood results can be returned the same day and to minimize disruption of a patient's daily routine. Risks include an increased possibility of multiple births, ovarian cysts, and in rare cases, ovarian hyperstimulation syndrome (OHSS). Symptoms of OHSS include nausea, vomiting, bloating, weight gain, laborious breathing, and severe pain in the pelvis, chest, or abdomen.
Clomiphene, Serophene®, Repronex®, Follistim®, Bravelle™, Pergonal®, Gonal-F®, Metformin®, and Parlodel are some of the most commonly prescribed fertility drugs. They are used to treat women with infrequent periods, infrequent ovulation, polycystic ovary syndrome, unexplained infertility, and high prolactin levels.

Intrauterine Insemination (IUI) is a relatively simple procedure which facilitates fertilization by placing sperm in a woman's uterus. During IUI, a man's sperm is inserted into the uterine cavity through a soft tube called a catheter. The procedure is scheduled close to when a woman begins ovulating. IUI increases the likelihood of pregnancy, because sperm are able to bypass the cervix and a higher number have a chance to get closer to the egg.
IUI is typically performed when the sperm and cervical mucus are incompatible, when a man is impotent or suffers from premature ejaculation, or when there is a low sperm count or motility. The woman must have at least one open fallopian tube to be able to use IUI. Sperm used in the procedure may come from a partner or from a donor.
Since the IUI procedure does not require medications or anesthesia, it is a popular option with many couples. Intrauterine insemination is also a less invasive and more affordable alternative to IVF for couples with unexplained infertility or mild male factor infertility. The success of IUI, as with other artificial insemination procedures, is dependent on the health of both the woman's body and the man's sperm.

A significant technological advancement, laparoscopic surgery allows us to view the inside of a patient's abdomen, including the reproductive organs, through a small, tube-like device known as a laparoscope. The laparoscope functions like a lighted telescope inside the patient's body. It allows us a clearer view of the internal organs in a minimally invasive way and can even remove ovarian cysts, adhesions in the pelvic area, or an obstructed fallopian tube, called a hydrosalpinx. Endometriosis is a disease commonly found in patients with infertility and pelvic pain. The only way to diagnose endometriosis is by looking into the abdomen. Treatment can be done at the same time with laser or cautery.
Because only small incisions are used, most patients return home the same day with a recovery period of less than a week. Dr. Asmar, Dr. Durso and Dr. Sarajari are highly skilled in laparoscopic surgery and have performed hundreds of procedures. We perform most of our surgeries in our Surgery Center at the Annandale location. This allows for more personalized service and convenience.
Sometimes women who have previously had their tubes tied would like another chance to become pregnant; the Tubal Ligation Reversal procedure gives these women a second chance.
This is a very delicate procedure that requires a microscope and an experienced hand. Sutures about the size of a human hair are used to re-align the tubes. The procedure is done as an outpatient and you can go home the same day with a small incision on the lower abdomen. The success rates are based on the amount of tube that is remaining for repair and the woman’s age. Many times there is no insurance coverage for tubal reversal surgery. By having the surgery in our outpatient center we can control the cost and offer tubal ligation reversal to you for a cost of $6900.
